How fax submission works
Fax is the most common submission channel for VLS Pharmacy. The prescription must be written on a state-issued prescription pad or a prescriber-created prescription form — VLS Pharmacy does not supply forms, and state boards require patient-specific compounding prescriptions to meet standard requirements. The fax number is 855-491-0799. VLS Pharmacy recommends eFax or FaxBurner for paperless fax, and provides a fax cover sheet checklist on newdrugloft.com/prescribers/fax-order/ to confirm all required fields are included before sending.
Required fields on every fax include patient full name, home address, phone number, date of birth, email address (for tracking notifications), and known drug allergies. The prescription must include the compound name (or the closest ingredient in your system plus pharmacy notes specifying the full formulation), dosing instructions (SIG), quantity and volume, and any relevant pharmacy notes. Missing patient demographics or an incomplete SIG are the most common reasons for delays or follow-up calls from the pharmacy.
