Second, your clinic's expected order volume and relationship with your account manager. Third, the ancillary cost structure: whether UPS shipping, the $5 optional signature fee, and card processing are bundled into the drug rate or billed separately. Because none of these are published, the same medication can be priced differently for different clinics, which is why a written, itemized rate from your account manager matters more than a verbal headline.
Treat the clinic agreement as a negotiation, not a take-it-or-leave-it. Ask for per-vial cost in writing on your top three SKUs, ask whether that rate is fixed or changes at renewal, and ask what is included versus billed separately. A clear agreement answers all three; a vague one leaves landed cost undefined until your first invoice.
