Tailor Made Compounding registration requirements

What you need first

• Clinic informationClinic name, phone, website, billing address, and shipping address (with copy-from-billing option). Optional checkboxes for Biote-certified prescriber and telehealth practice. Mandatory selects for number of providers (1, 2–4, 5+), non-provider employees, patients (<200, 201–500, 500+), and locations (Telehealth Only, 1, 2, 3+).
• Office contactBest office contact name, position, and email — the person Tailor Made will reach during review. This should be someone who can supply missing documents quickly.
• Prescriber credentials per providerFor each prescriber: first and last name, NPI, license type (MD, NP, PA, DO, naturopath, dentist, podiatrist, CRNA), independent prescriptive authority (yes/no), license number, license state, upload of U.S. medical license, and DEA license status. The form includes multiple prescriber slots; more can be added later in the portal.
• Communication and fulfillment defaultsOrder notification email and opt-in stages (In Dispensary, In Shipping, Has Shipped). Defaults for whether prescriptions bill to clinic or patient and ship to clinic or patient — adjustable later in the portal per Tailor Made's form notes.

How access works

   Collect clinic entity detailsGather legal clinic name, both addresses, phone, website, and practice-scale answers before opening the form so you do not lose progress mid-session.

   Prepare prescriber license filesExport or scan each prescriber's U.S. medical license for upload. Confirm NPI, license number, and state match the document exactly.

   Assign an office contactDesignate the best office contact with authority to respond to Tailor Made review questions within one business day.
4. 04
   Submit and monitor emailComplete Review & Submit on register.aspx. Watch the office contact inbox — and accountrequest@tailormadecompounding.com if you sent supplemental documents.
