The line items to confirm in writing
Work through each of these with your Promise account manager and get the answer in your agreement, not in conversation. Shipping and handling: is it per order, per patient shipment, flat, or included in the wholesale drug rate — and does cold-chain add cost on GLP-1 lines? Payment processing: is a card or ACH fee passed to you on each transaction? Rush or expedited compounding: what triggers an added charge outside the standard five-day fulfillment window? Account minimums: is there a minimum order or monthly volume for wholesale tier pricing, and what happens if you miss it? Returns, cancellations, and reships: who absorbs cost when an order is wrong, delayed, or temperature-compromised?
Once you have those answers, recompute per-vial landed cost by spreading every applicable fee across the vials in a typical order. The headline wholesale rate almost always looks better than the all-in number; your patient pricing and margin model should be built on the all-in figure. If a fee is variable, ask for the typical case so you can plan, not just the best case.
